Child Health Records: Tools for Caregivers

  • WellCare Tracker
    WellCareTracker™ is a web-based tool that assesses the completeness and timeliness of routine preventive health services received by children. Enter the child's health information, and WellCareTracker™ determines which immunizations and/or services are up-to-date, currently due, or overdue based on the American Academy of Pediatrics' recommendations.

  • Sample Letters to Help Caregivers Communicate With Parents and Doctors
    These sample letters, available on the Pennsylvania Early Childhood Education Linkage System (ECELS) web site, will help caregivers to facilitate communication with parents and doctors about child health records.

  • Health Form- Infant/Toddler (birth to 36 months) Development and Routine
  • Health Form- Preschool (3-5 years) Development and Routine
    As a basis for meeting a child's health, behavioral and social needs in the child care setting, the file for each child should include a history form that is completed by the parent/guardian upon admission, preferably with assistance from the staff.   • Injury Report Form
    It is important that all child care programs record any injury that requires first aid, such as a bandage or a cold compress. Planning for injuries, reporting injuries and eliminating hazards or conditions that cause injuries should be part of the overall plan to keep children safe while they are in child care. And by tracking injuries that happen, programs can look for patterns caused by hazardous conditions and identify problem areas before they cause serious injury. This form is to be used to ensure accurate tracking and appropriate follow-up of all serious incidents that occur in child care.
    • This form was developed by California Child Care Health Program staff.
      If you discover a child's injury, such as bruising, cuts or burns, and suspect child abuse, you are required to file a report with your local child protective services agency.

  • The California Childcare Health Program:
    • Fact Sheets for Families: Written with the needs of families in mind and provide easy-to-understand information on issues that arise in the home, such as teeth, avoiding burns, and good nutrition.
    • Health and Safety Notes: Written for child care professionals and focus on issues that frequently arise in child care, such as how to handle biting, toileting, specific illnesses, etc.
    • Survival Tips Posters: Attractive mini-posters for the child care setting. They provide health and safety tips, such as step-by-step instructions on proper diapering.
    • Training Curricula: In-depth materials suitable for use in training health professionals to work in early care and education settings, or to provide continuing education for child care professionals.


  • Brochure: Help Me Be Successful, Help Me Be Healthy: Great resource that explains what parents and families can do to encourage healthy development. This Curriculum Guide details how health educators and child care health consultants can best present the material to parents and families.
